Tomasz Ostrowski, Doctor of Medical Sciences, in the course of his individual specialist medical practice, takes every care to protect personal data by applying the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (“GDPR”). For what purposes and what personal data are processed?

The personal data of persons who contact Tomasz Ostrowski, MD, PhD, conducting an individual specialist medical practice (in particular data from the contact form on the website) will be processed for purposes depending on the reason for contacting the doctor – in particular, in order to respond to your message or enquiry. Providing personal data is voluntary; however, without it we will not be able to reply to your message. The basis for processing personal data is Article 6(1)(f) of the GDPR, i.e. the legitimate interest of the Controller in the form of answering questions and establishing contact with a person who seeks contact using the data provided on the website. • the right to data portability: where legally permitted, you may request that the personal data provided to us be returned to you or transferred to a third party.

Tomasz Ostrowski, MD, PhD, takes the utmost care to provide you with the fullest possible protection of personal data in accordance with applicable law. If, despite this, you consider that your personal data is being processed unlawfully, you have the right to lodge a complaint with the supervisory authority, which in Poland is the President of the Personal Data Protection Office (Prezes Urzędu Ochrony Danych Osobowych).
